CORE Licensed Provider – Live Well Homecare

The licensed CORE provider will deliver face-to-face mobile services to individuals with mental health and substance use disorders in a community setting.

Key responsibilities include:

Providing person-centered, goal-oriented support and solution-focused interventions

Identifying treatment strategies to reduce symptoms and environmental stressors

Offering supportive counseling, behavioral management, and problem analysis to individuals and families

Promoting strengths-based planning and treatment approaches

Supporting recognition and management of triggers to maintain community stability

Implementing evidence-based psychotherapeutic interventions

Documenting accurately in compliance with standards

Participating in supervision, team meetings, and professional development

Required qualifications include licensure (e.g., LMSW, LCSW, LMHC), minimum three years of community-based experience, excellent communication skills, cultural responsiveness, and proficiency with electronic health records. Bilingual candidates and those with lived recovery experience are encouraged to apply. A valid NY driver’s license and a clean record are necessary.
