Overview
Manage and coordinate interdisciplinary teams to provide hospice care to patients and families, ensuring clinical and quality improvement initiatives comply with regulatory standards. Demonstrate excellence in clinical practice and customer service, working under general supervision.
Responsibilities
- Assess patients’ medical, physical, social, emotional, and spiritual needs using an interdisciplinary approach.
- Provide support and guidance to families in line with the hospice care plan and policies.
- Participate in team meetings, management discussions, and committees to promote collaboration, develop procedures, and enhance quality and training.
- Ensure regulatory compliance through audits and reviews of files and processes.
- Contribute to quality improvement and educational initiatives to maintain professional standards.
- Support achievement of hospice care goals, including managing operations, caseloads, and productivity targets.
- Perform managerial duties such as staff training, performance evaluation, budgeting, hiring, and disciplinary actions.
- Lead and facilitate interdisciplinary team efforts to deliver comprehensive hospice services.
- Participate in special projects and perform other assigned duties.
Qualifications
Licensed Registered Nurse and Nurse Practitioner in New York State preferred, with a Bachelor’s in Nursing or equivalent experience. Minimum of three years hospice clinical experience required, with clinical management skills preferred. Effective communication skills and proficiency in MS Office are desirable.
