Role: Clinical Pharmacist Specialist / Float
Responsibilities:
- Provide advanced clinical pharmacy services across various hospital units and specialties.
- Optimize medication therapy and perform order verification, therapeutic drug monitoring, and medication reconciliation.
- Collaborate with interdisciplinary teams through rounding and shared decision-making.
- Support evidence-based protocols and promote medication safety.
- Educate staff and patients to enhance understanding of medication use.
- Float to areas of highest need within the health system, ensuring comprehensive coverage.
