Role: Registered Nurse in the Transitional Care Unit, providing direct patient care within a fast-paced, multidisciplinary environment.
Responsibilities:
- Assess, diagnose, plan, implement, and evaluate patient care according to nursing standards.
- Promote patient and family-centered care, including education on illness management, medication, and post-discharge needs.
- Collaborate with physicians, therapists, case managers, and pharmacists to ensure optimal patient outcomes.
- Develop and communicate individualized care plans, addressing physical, psychosocial, and emotional needs.
- Participate in activities that enhance patient safety, quality, and compliance, and engage in ongoing professional development.
Qualifications & Skills:
- Associate’s or Bachelor’s Degree in Nursing; RN license required.
- Experience is preferred but not mandatory; strong critical thinking and assessment skills are essential.
- Ability to work collaboratively in a team, demonstrate adaptability, and prioritize patient safety.
