Clinical Nurse – Live Well Homecare

Role Overview:

This position involves providing direct patient care in a 24/7 Cancer Care Clinic, including assessment, diagnosis, planning, implementation, and evaluation within nursing standards. The role emphasizes patient and family-centered care, education on illness management, medication administration, and post-treatment needs. It requires participating in safety, quality, and regulatory activities, and collaborating with a multidisciplinary team to develop and communicate individualized care plans that address physical, psychosocial, and emotional needs.

Key Responsibilities:

Promote a healing environment through education and support to patients and families.

Participate in safety and quality initiatives, maintaining professional development.

Assess patient preferences, barriers, and needs across emotional, cultural, and spiritual domains.

Proactively communicate and coordinate care across the continuum.

Qualifications & Skills:

Minimum education: Nursing Diploma or Associate’s Degree; preferred Bachelor’s Degree in Nursing.

Licensure as an RN; no prior experience required, but 2 years of nursing experience is preferred.

Ideal candidates will have oncology experience.

Ability to work flexibly across day, night, weekend, and weekday shifts, including PRN shifts.
