Clinical Nurse – Live Well Homecare

Role Overview: Provides direct patient care, including assessment, diagnosis, planning, implementation, and evaluation within established nursing standards.

Key Responsibilities:

Promotes patient and family-centered care in a healing environment.

Educates patients and families on illness management, post-treatment care, and medication administration.

Participates in activities that enhance patient safety, quality, and regulatory compliance.

Develops, implements, and documents individualized care plans with collaborative input.

Assesses physical, psychosocial, and emotional needs, considering patient preferences, values, and cultural factors.

Ensures effective communication across care continuum.

Works in a safety-sensitive role requiring alertness and safe work practices.

Qualifications & Skills:

Education: Nursing Diploma/Associate’s degree in Nursing; Bachelor’s preferred.
Licenses & Certifications: RN license required.
Experience: No experience required; less than 2 years preferred.

Additional Information: The position involves shifts of 8 or 10 hours with a 40-minute call response time. The role emphasizes safety, professional development, and patient-centered care to support a high standard of nursing practice.
