Certified Lymphedema Therapist (CLT) – Outpatient Rehab

Role Overview: A Physical Therapist assesses, plans, and implements rehabilitative programs to improve mobility, reduce pain, and prevent deformities in patients with disease or injury. The role involves providing care across the full continuum, ensuring compliance with regulatory standards, and promoting quality and customer service.

Key Responsibilities:

  • Treatment Planning: Develops and revises treatment plans, selects appropriate activities, and offers patient/family education and discharge instructions.
  • Patient Assessment: Collaborates with physicians to evaluate and establish treatment goals, plans, and discharge strategies, ensuring realistic outcomes.
  • Documentation: Records evaluations, treatment sessions, education, and discharge details per departmental protocols.
  • Professional Conduct: Maintains ethical standards, confidentiality, punctuality, and effective communication. Participates in meetings and process improvements.
  • Customer Service & Teamwork: Applies AIDET principles, fosters a respectful and positive environment, and promotes teamwork and trust.
  • Professional Development: Completes mandatory education, maintains licensure, and assists in training new staff.

Qualifications & Skills:

  • Bachelor’s, Master’s, or Doctorate in Physical Therapy.
  • Licenses: Physical Therapist (PT) and BLS certification.
  • Effective communication, customer-focused approach, teamwork skills, and basic computer proficiency.
