We are seeking a full-time Family Nurse Practitioner to provide comprehensive, community-focused primary care in Hampton Roads.
Provide evidence-based care to adult patients, focusing on prevention, chronic, and acute conditions.Collaborate with patients and care teams to develop personalized care plans respecting diverse backgrounds.Perform physical exams, diagnostic testing, medication management, and coordinate onsite lab and pharmacy services.Drive population health initiatives, health promotion activities, and participate in organizational committees.Ensure quality, patient satisfaction, and achievement of clinical outcomes.Requirements include board certification in primary care, current Virginia license, 3-5 years of primary care experience, EMR proficiency, and CPR certification.
