Role Overview
The Care Partner provides personalized, relationship-based care to residents in a senior living environment, focusing on enhancing residents’ quality of life through daily support and engagement.
Key Responsibilities:
- Deliver hands-on care and emotional support according to each resident’s Individualized Service Plan (ISP)
- Partner with residents using prompting, cueing, and reminders to promote independence
- Observe, document, and communicate changes in residents’ conditions
- Support daily activities, programs, and meaningful engagement initiatives
- Assist with dining, housekeeping, and laundry services
- Uphold safety, dignity, and Resident Rights at all times
- Maintain regular communication with families and care leadership
- Follow infection control, OSHA, and safety standards
Qualifications & Skills:
- High School Diploma/GED; age 18+
- Experience working with seniors preferred
- Compassionate, patient, dependable, with strong communication and organizational skills
- Ability to remain calm and make sound decisions
- CNA/HHA certification as required by state (NJ required)
