Role: Care Manager (Registered Nurse) responsible for coordinating and overseeing clinical care for senior residents in a rehabilitation and long-term care setting.
Key Duties:
- Complete resident assessments and develop, implement, and regularly update individualized care plans.
- Monitor resident progress and ensure safe, high-quality clinical outcomes.
- Collaborate with an interdisciplinary team to align care and support best practices.
- Provide education to residents and families regarding care, health needs, and expectations.
- Support discharge planning and continuity of care.
- Contribute to quality improvement and compliance initiatives.
Qualifications & Skills:
- Current Registered Nurse (RN) licensure.
- Strong clinical assessment, care planning, and monitoring skills.
