Enhance patient care quality and outcomes

Improve patient and family healthcare experience

Reduce preventable hospitalizations and costs

Responsibilities include conducting exams, diagnosing, prescribing, managing health conditions, and providing preventive care.
They work autonomously, making clinical decisions within scope, and collaborate with facility leadership and interdisciplinary teams.
The role involves ensuring compliance, maintaining documentation, participating in quality initiatives, and promoting safety and health education.
Candidates need a relevant degree, at least 2 years of experience (long-term care preferred), and active APRN or PA licensure.
