APRN/PA – Post-Acute and Long-Term Care (PALTC) – Advanced Practitioner (AP)

The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is an APRN or PA responsible for independently managing patients in long-term care settings to enhance quality of care, improve patient outcomes, and reduce avoidable hospitalizations and costs.

Responsibilities include conducting physical exams, diagnosing, treating illnesses, prescribing medications, and providing preventive care. The AP works collaboratively with facility leadership and interdisciplinary teams, ensuring compliance with regulations, maintaining accurate documentation, and participating in quality improvement initiatives.

They oversee infection control, health education, and promote safety standards. The role requires strong clinical decision-making, excellent communication skills, proficiency with EHR and telemedicine tools, and the ability to work independently in a fast-paced environment. Qualifications include a relevant degree, at least 2 years of experience (long-term care preferred), and appropriate licensure/certification. The position is full-time, in-person, with benefits provided.
