APRN/PA – Post-Acute and Long-Term Care (PALTC) – Advanced Practitioner (AP)

The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is an in-person, full-time role for an APRN or Physician Assistant with at least 2 years of experience, preferably in long-term care.

  • They will independently manage patients’ comprehensive medical needs in PALTC settings, focusing on improving care quality, patient outcomes, and reducing hospitalizations and costs.
  • Responsibilities include conducting assessments, diagnosing, treating, prescribing, and developing care plans, collaborating with facility teams, ensuring compliance, and participating in safety protocols.
  • The role requires strong communication, clinical decision-making, and proficiency with EHR and telemedicine tools.
  • Qualifications include a relevant degree, board certification, and licensure as an APRN or PA.
