APRN/PA – Post-Acute and Long-Term Care (PALTC) – Advanced Practitioner (AP)

The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is a full-time, in-person role for an APRN or Physician Assistant with at least 2 years of experience, preferably in long-term care.

They work independently, managing comprehensive patient care in PALTC settings to improve outcomes, enhance patient and family experience, reduce hospitalizations, and lower costs.

Responsibilities include conducting physical exams, diagnosing and treating illnesses, developing care plans, providing preventive services, and collaborating with facility staff and families.

The AP ensures compliance with regulations, maintains accurate documentation, participates in quality initiatives, and promotes health and safety protocols.

Essential skills include strong clinical decision-making, excellent communication, proficiency with EHR and telemedicine, and the ability to work independently.

Qualifications include a relevant degree, board certification, APRN or PA licensure, and long-term care experience preferred.
