APRN/PA – Post-Acute and Long-Term Care (PALTC) – Advanced Practitioner (AP)

The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is an in-person role for an APRN or Physician Assistant with at least 2 years of experience, preferably in long-term care. The AP works independently, collaborating with facility leadership and interdisciplinary teams to manage patients’ comprehensive medical needs. Key responsibilities include conducting assessments, diagnosing, treating, and prescribing, as well as developing care plans to improve outcomes, enhance patient and family experience, reduce hospitalizations, and control costs. The role emphasizes self-directed practice, compliance, documentation, health and safety oversight, and effective communication. Proficiency with EHR and telemedicine is required.
