The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is an APRN or Physician Assistant who works independently to manage patients in PALTC settings.
Provide comprehensive patient assessments, diagnose, treat, and develop care plans for acute and chronic conditions. Collaborate with facility leadership and interdisciplinary teams to ensure holistic care and effective communication with families and external providers. Ensure compliance with regulations, maintain accurate documentation, and participate in quality improvement initiatives. Work independently, make clinical decisions, and handle urgent situations confidently. Utilize EHR and telemedicine tools effectively, promote health and safety protocols, and educate staff and patients on health promotion. Qualifications include a relevant degree, at least 2 years of experience (preferably in long-term care), and active APRN or PA licensure.
