The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is an APRN or Physician Assistant providing independent, comprehensive medical care in long-term care facilities.
Responsibilities include:
- Conducting physical exams, diagnosing, treating illnesses, prescribing medications, and managing chronic conditions.
- Providing preventive care and health counseling.
- Collaborating with facility staff, leadership, and external providers to ensure holistic patient care.
- Maintaining compliance with regulations, accurate documentation, and participating in quality initiatives.
- Overseeing infection control, health promotion, and safety protocols.
- Working independently with strong clinical decision-making and communication skills.
- Utilizing EHR and telemedicine tools.
