The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is an independent APRN or PA responsible for managing patients in PALTC settings to enhance care quality, patient outcomes, and experience, while reducing hospitalizations and costs.
Key duties include conducting physical exams, diagnosing and treating illnesses, prescribing medications, and providing preventive care. The AP works autonomously within scope, collaborating with facility leadership and interdisciplinary teams, and building strong relationships with patients and families.
They ensure compliance with regulations, maintain accurate documentation, and participate in quality improvement. The role involves health and safety oversight, staff education, and utilizing EHR and telemedicine tools. Candidates must have a relevant degree, at least 2 years of experience (preferably in long-term care), and board certification.
