Role: Provides direct patient care in an ambulatory setting, collaborating with providers and care teams to develop and implement personalized care plans aimed at optimizing health outcomes.
Responsibilities:
- Assess and evaluate patients’ physical, psychosocial, and emotional needs using critical nursing skills.
- Follow clinical protocols and documentation standards in accordance with compliance guidelines.
- Support providers by documenting assessments and treatment plans accurately.
- Educate patients and families with verbal and written instructions, assessing their understanding and addressing questions.
- Assist in daily patient management, offering oversight and guidance to clinical staff.
Qualifications & Skills:
- Education: Nursing Diploma/Associate’s degree in Nursing; Bachelor’s degree preferred.
- Experience: 2-5 years; 5-10 years preferred for advanced roles.
- Licenses & Certifications: Active RN license, BLS certification.
- Competencies: Strong critical thinking, communication, and patient education skills; ability to work safely and alertly in a healthcare environment.
