This full-time Ambulatory Clinic RN Care Coordinator role focuses on providing patient-centered care for individuals with pelvic floor disorders, urinary incontinence, prolapse, and postoperative recovery.
Serves as a clinical liaison among patients, providers, staff, and community resources to ensure high-quality care.
Supports patient education, care planning, triage, and care navigation, balancing hands-on nursing and back-office duties, including nurse-led procedures and provider assistance.
Manages patient flow, coordinates care, and promotes self-management, especially for high-risk populations with multiple comorbidities.
Utilizes electronic health records for documentation, communicates with patients and families, and ensures compliance with standards.
Requires RN license, at least 2 years of experience, BLS certification, and a bachelor’s degree in nursing.
Offers a schedule of four 10-hour days with no weekends, supporting work-life balance.
