Role Overview:

Develops and documents individualized care plans in collaboration with the healthcare team, ensuring effective communication across the continuum of care. Qualifications:
- Education: Nursing Diploma/Associate’s degree; Bachelor’s preferred
- Licenses & Certifications: RN license
- Experience: No minimum required; 2+ years preferred for advanced roles
- Supervisory Experience: Not required
