Role Overview
The Nurse Navigator is a nursing specialist responsible for coordinating comprehensive patient care across the treatment continuum. They collaborate with patients, families, physicians, and interdisciplinary teams to ensure optimal care delivery.
Key Responsibilities
- Follow patients’ care journeys, providing clinical expertise and support
- Coordinate treatment plans and facilitate communication among care teams
- Assist with patient education and community outreach, including group facilitation
- Support patients and families emotionally and informationally throughout their care
Qualifications & Skills
- Graduate from an approved nursing school and hold an active RN license
- Minimum of 2 years related nursing experience, with preference for oncology background
- Required certification: BLS within 30 days of hire; additional certifications in oncology or chemotherapy are preferred
- Strong physical stamina to lift up to 50 pounds, stand, walk, and perform various physical tasks
