Acute Medicine Nurse – Live Well Homecare

Role Overview: Provides direct patient care activities including assessment, diagnosis, planning, implementation, and evaluation within established nursing standards. Promotes patient and family-centered care, educating on illness management, post-treatment care, and medication administration. Participates in activities that enhance patient safety, quality, and compliance, while engaging in ongoing professional development.

Key Responsibilities:

Develops and documents individualized care plans with clear goals, collaborating with the interprofessional team, patients, and families.

Uses critical nursing skills to evaluate physical, psychosocial, and emotional needs, considering patient values, cultural, spiritual, and specific needs.

Ensures effective communication of care plans across all care continuum points.

Works in a safety-sensitive role, maintaining constant alertness to ensure safety and quality care.

Qualifications & Skills:

Education: Nursing Diploma or Associate’s Degree; Bachelor’s preferred.

Licenses: Valid RN license.

Experience: No prior experience required; 2+ years preferred.

Skills: Strong assessment, communication, and critical thinking abilities; commitment to safety and patient-centered care.
