Job Summary
The Pharmacist ensures the delivery of safe, effective, and cost-efficient medication therapy, developing and monitoring evidence-based care plans focused on patient outcomes and quality of life. They assess medications for indication, safety, effectiveness, and adherence, and monitor laboratory parameters and documentation.
Key Responsibilities
- Provide pharmaceutical care tailored to the specific patient care area.
- Coordinate resources for timely, efficient, and appropriate care.
- Interpret, clarify, and correct physicians’ medication orders.
- Review medication profiles to prevent drug interactions, contraindications, and sensitivities.
- Evaluate prescribing, formulary, and therapeutic issues, suggesting alternatives as needed.
- Consult with medical staff on drug therapy and monitor lab results.
- Provide drug information to healthcare professionals and discharge medication counseling.
- Participate in rounds with medical teams and calculate drug dosages.
- Verify orders, dispense proper doses, and educate healthcare providers.
- Prepare compounded and sterile medications using aseptic techniques.
- Manage supplies, supervise pharmacy staff, and ensure compliance with standards.
- Assist with departmental policy review and collaborate with healthcare teams to resolve issues.
