Advanced Practice Provider – Live Well Homecare

We are seeking a dedicated Nurse Practitioner (NP) or Physician Assistant (PA) to join our team in providing advanced patient care across clinic, dialysis, and hospital settings.

Key responsibilities include:

Conducting daily hospital rounds on nephrology patients

Collaborating with nephrologists and hospital teams to coordinate care

Managing follow-ups and chronic kidney disease (CKD) patients

Providing outpatient nephrology care as needed

Educating patients on kidney health and disease management

Requirements:

Current certification as an NP or PA

Knowledge of HIPAA and patient privacy standards

Nephrology experience preferred

Strong communication and teamwork skills
