The Medical Social Worker (MSW) provides psychosocial assessments and supports patients and families in the community receiving Home Care and Hospice services. An integral team member, the MSW helps patients navigate illness, plan long-term care, and connect to community resources.

Responsibilities encompass psychosocial assessments, social evaluations, providing resource referrals, facilitating communication with community agencies, participating in care planning, and supporting discharge processes.
Qualifications include a Master’s in social work, healthcare experience, strong communication skills, and valid driver’s license with reliable transportation. Additional requirements are proof of flu vaccination, recent physical exam, and TB test.
