The licensed CORE provider will deliver face-to-face mobile services to individuals with mental health and substance use disorders in a community setting.
Key responsibilities include:
- Providing person-centered, goal-directed support and solution-focused interventions.
- Identifying strategies to minimize symptoms and environmental stressors.
- Offering supportive counseling, emotional management, and behavioral analysis to individuals and families.
- Facilitating participation in strengths-based treatment planning.
- Helping individuals recognize and respond to triggers to remain in their community.
- Implementing evidence-based therapeutic modalities.
- Supporting families and significant supports affected by or influencing the individual’s condition.
- Maintaining accurate documentation per standards and participating in supervision and training.
Qualifications include:
- Licensed mental health professional with at least 3 years of community-based experience.
- Excellent interpersonal and communication skills; culturally responsive approach.
- Ability to collaborate with health professionals and proficiency with electronic health records.
- Knowledge of recovery principles and relevant housing programs.
- Bilingual skills and lived experience encouraged.
- Valid NY driver’s license and a clean driving record.
