Family Care APP (NP or PA)

The Advanced Practice Provider (APP) provides comprehensive outpatient primary care under supervision at the Prineville Clinic, serving patients of all ages.


  • Responsibilities include evaluation, diagnosis, treatment, documentation, and coordination of care within a Medical Home model.
  • Creates integrated treatment plans incorporating behavioral health, pharmacy, therapy, nutrition, and surgery; monitors patient progress and refers to specialists as needed.
  • Works a minimum of 36 hours of patient contact weekly, participates in shared call rotation including evenings, weekends, and holidays.
  • Collaborates with multidisciplinary teams to deliver evidence-based, personalized care, ensuring quality outcomes and patient satisfaction.
  • Maintains documentation, billing, and compliance with policies and licensing requirements.
  • Engages in ongoing education, quality improvement, and adheres to standards of professionalism and confidentiality.


    Qualifications include a master’s degree in Physician Assistant Studies or Nursing, valid licensure, certifications, and experience in outpatient family medicine. Preferred candidates have 2+ years of experience, familiarity with Medical Home models, and Epic EHR experience. The role offers full-time employment with competitive salary and benefits, requiring relocation to Prineville, OR.
