Seeking a Speech Language Pathologist for our Home Care team to provide personalized assessments and treatments in patients’ homes, focusing on communication, cognition, and swallowing to promote safety and independence.
Responsibilities include interpreting medical prescriptions, conducting evaluations, developing and adjusting evidence-based treatment plans, documenting progress, educating patients and caregivers, and collaborating with interdisciplinary teams.
Qualifications: Master’s degree in Speech-Language Pathology, CCC or CFY, NY State license, CPR certification; experience with dysphagia and neurogenic disorders preferred.
