Speech Language Pathologist (Home Health) (Part time/PRN)

We are offering a Part-time/PRN opportunity for a licensed Speech Language Pathologist (SLP) to assess, diagnose, treat, and prevent communication and swallowing disorders in patients’ homes.

Responsibilities include conducting initial assessments, developing individualized treatment plans, collaborating with the care team to ensure continuity of care, and documenting patient progress in the health information system.

The role involves working with patients who have cognitive-communication and swallowing issues resulting from strokes, brain injuries, illnesses, surgeries, injuries, or dementia. Candidates must have current SLP licensure and a valid driver’s license.
