Seeking a Speech Language Pathologist for our Home Care team to provide personalized evaluation and treatment in patients’ homes, focusing on communication, cognition, and swallowing to promote safety and independence.
Responsibilities include interpreting medical prescriptions, conducting assessments, developing evidence-based care plans, adjusting treatments, documenting progress, and educating patients and caregivers.
Qualifications: Master’s degree in Speech-Language Pathology, CCC or CFY, NY license, and CPR certification; experience with dysphagia and neurogenic disorders is preferred.
