The CORE licensed provider offers
direct, face-to-face mobile services in community settings for individuals with mental health and substance use disorders.
Key responsibilities include
– delivering time-limited, goal-oriented, and solution-focused support
– utilizing person-centered, strengths-based treatment approaches
– providing supportive counseling, emotional and behavioral management, and problem analysis
– helping individuals recognize and manage triggers to stay in their community
– implementing evidence-based interventions
– supporting families and significant supports
– maintaining accurate documentation
– participating in supervision, meetings, and professional development
Required qualifications include
– valid professional license (e.g., LMSW, LCSW, LMHC, RN, Psychologist, LMFT, CRC)
– at least 3 years of community-based experience
– strong interpersonal, communication, and collaboration skills
– commitment to culturally responsive care
– proficiency with electronic health records
– knowledge of relevant programs (Health Homes, HARP, SPA)
– valid NY driver’s license and clean record
– bilingual or lived experience encouraged
