Family Nurse Practitioner – Live Well Homecare

We are seeking a dedicated Family Nurse Practitioner (FNP) to provide comprehensive, patient-centered care across all ages in a Level I Trauma Center setting.

Conduct physical exams, diagnose, and manage acute and chronic conditions.

Develop individualized care plans and provide patient education.

Perform diagnostic tests, administer treatments, and collaborate with healthcare teams.

Specialize in areas such as toddler, nursing home, and medical-surgical care.

Maintain accurate documentation and participate in clinical research.

Utilize skills in psychopharmacology, assessment, and emergency procedures.
