The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is an APRN or Physician Assistant who independently manages comprehensive patient care in PALTC settings.
Goals include improving care quality, patient and family experience, reducing preventable hospitalizations, and lowering costs.
Responsibilities encompass conducting examinations, diagnosing, treating illnesses, developing care plans, providing preventive services, and collaborating with facility staff and external providers.
They work autonomously, making clinical decisions within scope, ensuring compliance, maintaining accurate records, and participating in quality initiatives.
Additional duties involve promoting health and safety, educating staff and patients, and utilizing electronic health records and telemedicine tools.
Qualifications include a relevant degree, at least two years of experience, board certification, and licensure as an APRN or PA.
