We are seeking a compassionate Nurse Practitioner to join our post-acute care team, providing patient-centered care in the community. This full-time role offers autonomy with strong clinical support in a collaborative, physician-led environment.
- Manage and coordinate care for your own patient panel with comprehensive onboarding and ongoing training
- Collaborate with nursing staff, physicians, and specialists
- Maintain accurate electronic medical records and communicate effectively within the team
