The Advanced Practice Provider (APP) provides comprehensive outpatient medical care across all ages under supervision, including evaluation, diagnosis, treatment, and documentation in a Medical Home model.
They coordinate services with behavioral health, nursing, pharmacy, and other teams to optimize patient outcomes and develop integrated treatment plans. The role involves patient education, medication management, diagnostic interpretation, and participation in after-hours call rotation.
Located at Prineville Clinic, this full-time position requires a minimum of 40 hours/week with scheduled patient contact, and shared call duties. The APP supports quality improvement initiatives, collaborates with multidisciplinary teams, and adheres to all legal and organizational standards.
Qualifications include a Master’s in Physician Assistant Studies or Nursing, valid licensure, certifications, outpatient experience, and proficiency with EHR systems. Preferred candidates have 2+ years’ family medicine experience and familiarity with Medical Home models. Benefits include health, dental, vision, retirement, and paid time off.
