Nurse Practitioner – Geriatric Primary Care (SNF, LTC, ALF)

We are seeking a full-time Nurse Practitioner to provide primary care in Skilled Nursing Facilities, LTC, and ALFs within Minnesota.

Deliver comprehensive assessments, manage acute and chronic conditions, and develop treatment plans.

Order diagnostics, prescribe medications, and educate patients, families, and staff.

Collaborate with care teams to ensure seamless, patient-centered care.

Maintain accurate documentation and support quality initiatives.

Qualifications: Minnesota NP license, board certification, and preferably 1+ year of geriatric or post-acute care experience. Strong communication and teamwork skills required.
