Nurse Practitioner – Geriatric Primary Care (SNF, LTC, ALF)

We are seeking an experienced Nurse Practitioner to provide comprehensive primary care in Skilled Nursing Facilities, Long-Term Care, and Assisted Living Facilities.

Perform patient assessments, diagnose, manage acute and chronic conditions, and develop treatment plans.

Order and interpret diagnostics, prescribe medications, and educate patients, families, and staff.

Collaborate with care teams to ensure seamless care delivery and support quality improvement initiatives.
