We are seeking a compassionate and skilled Nurse Practitioner or Physician Assistant to join our outpatient diabetes clinic in Keene, NH.
Conduct patient visits, develop individualized care plans, and focus on disease prevention and glycemic control. Collaborate with endocrinologists, educators, dietitians, and nursing staff to provide holistic care. Educate patients and families on diabetes management, lifestyle changes, and medication use. Order and interpret labs, adjust medications, and monitor treatment outcomes. Participate in quality improvement initiatives and support program growth. Qualifications: NP or PA with NH licensure and national certification. Experience in endocrinology, diabetes, or primary care preferred.
