Nurse Practitioner – Geriatric Primary Care (SNF, LTC, ALF)

We are seeking an experienced Nurse Practitioner to provide comprehensive primary care in Skilled Nursing Facilities, LTC, and ALFs.

Perform assessments, exams, and chart reviews.

Manage acute and chronic conditions, order diagnostics, and develop treatment plans.

Prescribe medications and educate patients, families, and staff.

Collaborate with care teams to ensure seamless care delivery.

Maintain accurate documentation and support quality initiatives.

Qualifications: Minnesota NP license, board certification, 1+ year experience preferred, strong geriatric and primary care skills.
