Nurse Practitioner – Geriatric Primary Care (SNF, LTC, ALF)

We are seeking an experienced Nurse Practitioner to provide comprehensive primary care in Skilled Nursing Facilities, Long-Term Care, and Assisted Living Facilities.

Perform patient assessments, diagnose, and manage acute and chronic conditions.

Order, interpret labs, develop treatment plans, and prescribe medications within guidelines.

Educate patients, families, and staff to promote health and prevention.

Collaborate with care teams to ensure seamless, quality care delivery.

Maintain accurate documentation and support quality improvement initiatives.

Qualified candidates will hold a Minnesota Nurse Practitioner license, be board-certified, and preferably have over a year of experience in a skilled nursing or similar setting.
