Nurse Practitioner – Live Well Homecare

We are seeking a full-time Nurse Practitioner in Columbus, IN, to join a leading clinic focused on hormone therapy, athletic recovery, and wellness.

Conduct comprehensive patient assessments, develop and manage personalized treatment plans, and provide patient education.

Administer injections/IV therapy, monitor progress, and adjust therapies as needed.

Collaborate with a clinical team to ensure cohesive care and maintain high patient satisfaction.

Qualifications include an Indiana NP license and experience in hormone therapy or functional medicine preferred.
