Partner with patients, families, and healthcare teams to address social and emotional needs impacting medical care, recovery, and transitions.
Provide comprehensive social work services, including assessments, treatment planning, crisis intervention, discharge planning, and resource referrals.
Coordinate care across settings, ensuring safe discharges and effective resource utilization.
Offer specialty consultation, psychosocial support, case management, and referrals, including compliance with mandatory reporting laws.
Requirements include an MSW degree, one year of healthcare social work experience, and appropriate state licensure or credentials within 90 days of hire.
