LTAC RN (High-Acuity/Vent Care)
Provide high-acuity nursing care for medically complex, ventilator-dependent patients in a long-term acute care setting. Deliver comprehensive assessments and manage ongoing patient care with a focus on safety, quality, and clinical excellence.
Key Responsibilities
- Perform thorough assessments and develop/implement patient care plans
- Administer medications and therapies per clinical standards
- Provide ventilator and tracheostomy management and monitoring
- Manage wound care and other complex clinical needs
- Continuously monitor patient status and respond to changes
- Collaborate with an interdisciplinary team to support safe outcomes
- Document all care accurately in the EMR and follow safety, regulatory, and infection control requirements
Qualifications / Skills
- RN licensure with experience in LTAC or comparable high-acuity care
- Strong skills in ventilator/trach care, assessment, and ongoing monitoring
- Competence with medication administration, wound care, and clinical documentation
- Ability to work effectively in a team and maintain strict infection prevention practices
