Registered Nurse Stroke Coordinator to lead stroke program coordination, evidence-based care planning, and quality improvement initiatives.
Key Responsibilities
- Coordinate and oversee implementation of stroke protocols across the care continuum.
- Provide patient and family guidance, ensuring clear understanding of stroke care and next steps.
- Collaborate with an interdisciplinary team to support safe, timely, and effective care.
- Collect, analyze, and report stroke program data to track performance and outcomes.
- Educate and support staff on stroke best practices and protocol requirements.
- Support accreditation and quality standards through documentation and process improvement.
- Monitor key performance indicators (KPIs) and drive continuous improvement initiatives.
Qualifications & Skills
- Current RN licensure with strong clinical expertise in stroke care.
- Demonstrated ability to lead program coordination and quality initiatives.
