Key Responsibilities
- Perform in-home assessments and evaluate patient needs.
- Triages urgent concerns and initiates appropriate nursing interventions.
- Administer treatments and provide ongoing skilled nursing care.
- Coordinate care with an interdisciplinary team to support continuity of care.
- Accurately document clinical information in the EMR.
- Educate patients and families on care plans, health management, and instructions.
- Support safe transitions of care and follow established protocols.
Qualifications & Skills
- Valid nursing licensure and strong clinical judgment.
