Wellness Nurse (RN or LPN)
Provide resident-centered nursing care for individuals in assisted living and memory care. Conduct ongoing health assessments, monitor chronic conditions, and administer medications in accordance with established standards. Maintain accurate documentation and update care plans based on resident needs and clinical findings.
Collaborate with an interdisciplinary team to support safety, promote wellness, and enhance residents’ quality of life. Help foster an engaging, supportive environment while continually growing skills in a collaborative senior care setting.
Qualifications
- Current RN or LPN licensure
- Experience or strong commitment to senior care and person-centered practice
- Ability to assess residents, monitor conditions, and respond to changes
- Strong medication administration and documentation skills
- Team-oriented, compassionate communication
