Role: Provide evidence-based occupational therapy for adult and geriatric patients in their homes, promoting safety, independence, and quality of life.
Key Duties:
- Perform patient evaluations and develop, implement, and update individualized plans of care.
- Deliver one-on-one OT treatments focused on functional improvement and daily living skills.
- Collaborate with an interdisciplinary care team to support patient outcomes.
- Accurately document clinical findings and care plans in the EMR.
- Educate patients and caregivers on therapy goals, strategies, and home safety.
- Coordinate home visits and contribute to smooth care delivery in a fast-paced environment.
Qualifications/Skills: Occupational therapy licensure and strong clinical judgment, communication, and documentation skills; ability to work independently in home settings and partner effectively with interdisciplinary teams.
