Weekend Hospice RN provides compassionate skilled nursing care to hospice patients in their homes across the service area.
Key Responsibilities
- Perform comprehensive patient assessments and develop individualized care plans.
- Manage symptoms, coordinate and administer medications, and monitor patient response.
- Collaborate with an interdisciplinary hospice team to deliver coordinated end-of-life care.
- Use strong clinical judgment to make timely decisions and address changing needs.
- Support patients and families emotionally and clinically through end-of-life care.
- Practice autonomously while leveraging team support, resources, and guidance.
Qualifications & Skills
- Active RN licensure and hospice/home-care clinical competence.
- Excellent communication skills and ability to work effectively with patients, families, and the care team.
- Strong assessment, symptom management, and medication management skills.
- Compassionate, patient-centered approach and sound clinical decision-making.
