Community Health Nurse
Provide compassionate, faith-informed nursing care to seniors and vulnerable adults in home and community settings across the New York metro area. Conduct comprehensive health assessments, develop and implement individualized care plans, manage medications, and deliver patient and caregiver education.
Key Duties
- Assess patient needs and monitor health status.
- Create, update, and coordinate care plans.
- Administer and manage medications safely.
- Educate individuals and families on care and health maintenance.
- Support chronic disease management and promote wellness.
- Coordinate with an interdisciplinary team to ensure continuity of care.
- Connect patients to appropriate community and health services.
